The combat is extremely difficult, it requires you to manage the well-being of your character, and there is absolutely zero fast travel. They're very different, so it depends on what you like. Very hard is the normal game, but your guns don't do damage, while the enemy damage gets massively boosted. Other than that it's just like playing normally. Survival on the other hand tweaks things massively, so the game feels very different to play.
