I would check alignment between the cylinder and the barrel before firing. Pull the hammer to position 2, and spin the cylinder through at least 3 times to ensure it is unloaded, pull the hammer all the way back, and look down the barrel. I found that there was a ridge in the casting on the left side of the hammer. The hammer was also shifted to the left within the frame. There was a gouge in the left inside of the frame.
